การพัฒนาระบบสุขภาพทางเลือกเพื่อการส่งเสริมสุขภาพสตรีวัยกลางคน: วิเคราะห์สถานการณ์ในภาคเหนือ อิสาน และใต้ งานวิจัยเชิงคุณภาพเรื่องนี้มีเป้าหมายเพื่อวิเคราะห์สถานการณ์ระบบสุขภาพทางเลือกเพื่อการส่งเสริมสุขภาพสตรีวัยกลางคน ที่เป็นอยู่ใน 3 จังหวัด คือ สถานบริการสุขภาพในชุมชนเมือง และชนบทในจังหวัดเชียงใหม่ ของภาคเหนือ ขอนแก่นของภาคอิสาน และสงขลาของภาคใต้ ผู้ให้ข้อมูลหลักเป็นสตรีอายุ 40-59 ปี จำนวน 357 คน และผู้ให้บริการ 82 คน เก็บข้อมูลโดยการสังเกต การสัมภาษณ์ และการสนทนากลุ่มในช่วงเดือน มีนาคม 2542-กุมภาพันธ์ 2543 ตรวจสอบข้อมูลด้วยวิธีการตรวจสอบสามเส้า วิเคราะห์ข้อมูลโดยหลักการวิเคราะห์เนื้อหา ได้ข้อค้นพบ 7 ประเด็นที่สำคัญ ดังนี้ เรื่องของสุขภาพและการส่งเสริมสุขภาพดำเนินอยู่ในวิถีชีวิตอย่างเป็นองค์รวม การส่งเสริมสุขภาพเกิดขึ้นตามเหตุ ปัจจัย และเงื่อนไขชีวิตของสตรี และบริบทของสังคม ข้อมูลสุขภาพมีมากมายจนทำให้เกิดความสับสนทางความคิด และวิธีปฏิบัติเพื่อการส่งเสริมสุขภาพของสตรีวัยกลางคน ระบบสุขภาพทางหลักต้องพึ่งพาบุคลากรสุขภาพ ระบบสุขภาพทางเลือกต้องการแนวร่วมและความเป็นเจ้าของจากชุมชน สังคม กิจกรรมส่งเสริมสุขภาพสตรีวัยกลางคนที่มีอยู่ เน้นเรื่องอาหาร การปรับจิตใจ การออกกำลังกาย และการใช้ยา/สารเคมีต่าง ๆ มากที่สุด สตรีวัยกลางคนใช้วิธีการส่งเสริมสุขภาพทางเลือกมากกว่าทางหลักแต่ระบบประกันสุขภาพไม่ครอบคลุมกิจกรรมส่งเสริมสุขภาพ การสร้างความมั่นใจต่อการใช้วิธีการสุขภาพทางเลือก การสร้างความเข้มแข็งให้ชุมชนสตรี และการเพิ่มพื้นที่ทางสังคมแก่สตรีวัยกลางคนจะทำให้ระบบการพัฒนาสุขภาพทางเลือกเพื่อส่งเสริมสุขภาพสตรีวัยกลางคนดำรงอยู่ และขยายผลได้อย่างดี คำสำคัญ: สุขภาพทางเลือก ส่งเสริมสุขภาพ สตรีวัยกลางคน

Development of Alternative Health Systems to Promote the Health of Middle-Aged Women: A Situational Analysis in the North, Northeast, and South The purpose of this qualitative study was to explore the situation of alternative health systems for promoting the health of middle-aged women in 3 regions of Thailand. The study was undertaken at health services institutions and public places both in urban and rural areas of Chiang Mai (North), Khon Kaen (Northeast), and Songkhla (South), Three hundred and fifty seven women age 40-59 and eighty-two health discussions were used for data collection during March 1999-Februry 2000. Triangulation and content analysis were used for validation and analysis. Seven main themes emerged in describing an existing situation of alternative health promotion in the way of living and holism, 2) Factors and conditions within women’s life and social context that lead to health promotion practice, 3) Abundant and confusing information on the health promotion for middle-aged women, 4) A modern health system which is based on health profession, 5) An alternative health system which is independent and community based, 6) Alternative health practices which emphasize mostly on nutrition, mind control, exercise, and using chemical agent, and 7) Middle-aged women used alternative health systems more than modern medicine but health insurance undermined the health promotion activities. It is suggested that empowering women group in using alternative health systems and expanding their social arena are ways of developing alternative health systems to promote the health of middle-aged women.Keywords: alternative health, health promotion, middle-aged women
