This cross sectional study was conducted to identify the effective status of CPHCC and its determining factors. VHVs general characteristics, knowledge and attitude of VHV, characteristics of CPHCC and characteristics of village were measured as independent variables. The study was conducted during the first two weeks of April 1996, covering all 100 CPHCCs and VHVs in the same CPHCC of six districts in Nongbualumphu Province. Based on the 70 percentile value of score, CPHCC was divided into two group, effective and ineffective groups. The criteria in classifying these CPHCCs are drawn upon three dimensions of effectiveness

there are overall effectiveness, effective management and effective service of CPHCC. Relationship between each independent variable and effectiveness of CPHCC was tested by chi-square and the differences of knowledge and attitude of VHVs between two groups were tested by t-test. Only age, gender of VHVs and VHV selection process were found to have a significant association with the effectiveness of CPHCC while the other VHVs general characteristics had no significant association with the overall effectiveness, effective management and effective service of CPHCC. Supportive factors and special projects in the village had a significant association with the overall effectiveness, effective management and effective service of CPHCC. But the location of CPHCC and the accessibility of service had no significant association with the effectiveness of CPHCC. There was significant difference in knowledge of VHV between effectiveness and ineffectiveness of VHV and the similar pattern was found in effective service of CPHCC. But the attitude of VHVs had significant difference between the effective service and the ineffectiveness service of CPHCC. Based on the finding of this study, it is recommended that in selecting VHV, sex, age, selection process, knowledge and attitude of VHV should be well considered. It is recommended also, that in improving the effectiveness of CPHCC, consideration should be focus on improving management of information system, increasing training activities e.g. increasing blood pressure, urine sugar testing activities and strengthening of community revolving funds.
