บทคัดย่อ/คำอธิบาย :

Neisseria gonorrhoeae is the etiologic agent of gonorrhea. Rapid identification is essential for effective control. Fluorescein-conjugated wheat germ agglutinin (WGA-FITC) has been applied to confirm the identity of presumptive N. gonorrhoeae grew on primary genital culture on modified New York City medium (MNYC). This method was compared with fluorescein-conjugated monoclonal antibody (MAb-FITC). The selective medium of MNYC is more suggestive than modified Thayer-Martin medium (MTM) of both rapidity and economic. In 155 bacteria of pure culture including 72 gonococcal and 83 other nongonococcal Neisseria were tested. WGA-FITC gave 91.7% of sensitivity and 100% of specificity, however MAb-FITC still gave 100% of sensitivity and specificity, respectively. A total of 150 urethral of males and 151 of females endocervical were collected from a high-risk of symptomatic with a gonococcal disease which of 154 samples could grow on MNYC media and were presumptive gonococcal positive. The colonies were confirmed by both methods and the results were highly significant correlation (Mc Nemar test, X(2) =0.025, p>0.05). MAb-FITC and WGA-FITC showed 98.1% and 99.4% of sensitivity, respectively. Gram stain was also evaluted in clinical samples to show 92.9% of sensitivity and 98.6% of specificity, respectively. The most expensive test system was MAb-FITC when followed by WGA-FITC and carbohydrate utilization systems. The WGA-FITC appeared a rapid and accurate method to identify in vitro isolates of N. gonorrhoeae. WGA-FITC may be an alternative means suitable for using in laboratory in Thailand.
