| บทคัดย่อ/คำอธิบาย | : | The current problem in the Leprosy ControlProgram is low coverage of registered cases which isroughly estimated to be about 50% of the totalestimated cases in the country. There is a need forintensification of case finding activities in theimplementation of the Leprosy Control Program. The major objective of this study is to assessthe costs and benefits of different methods of casefinding activities : Active Case Detection and PassiveCase Detection, from the provider as well as patientperspectives. In this study benefits in terms of costsavings for early case detection were used to find outwhich method of case finding activities is better inthe sense that more early cases are detected. If thepatients are detected in the early stage that is beforedisability sets in, there will be substantial costsavings from provider as well as patient sides. The benefit/cost ratios are found out from threedifferent scenarios : Baseline scenario, ACD alonescenario and PCD alone scenario in low, median and highendemic areas of the country. The study shows that from the providerperspective, ACD alone scenario had the highestbenefit/cost ratio among the three scenarios. In the lowendemic areas the benefit/cost ratio for ACD alonescenario is 1.33 and the value in PCD alone scenario is1.25 but in high endemic areas these values vary toomuch: 10.38 and 1.37, respectively. It means that inthe high endemic areas ACD activity should be moreemphasized than PCD activity in terms of the early casedetection. From the patient perspective, in the lowendemic areas benefit/cost ratio for ACD is 34.86 butin the high endemic areas benefit/cost ratio for ACD is76.99 which is very much larger than PCD. The study concludes that ACD activities are moreemphasized than PCD activities especially in the highendemic areas. By doing economic evaluation, theprogram can identify which method of case findingactivities should be given more priority in differentendemic areas of the country in terms of analyzing thecosts for each method of case finding activities andcost savings for early case detection which is actingas benefit of the study. |
